Recent studies have shown that 78% of patients leaving a hospital ER are confused about their diagnosis, treatment, and follow-up care
and 20% of all inpatients experience an adverse event following discharge
The statistics on what happens to patients after they leave the hospital are not good. Forty percent of patients 65 or older have a medication error after they leave.
Adverse drug effects, therapeutic errors and nosocomial infections occur in 20% of the discharged patients and often to lead to readmissions not related to the initial condition.
One third of these adverse events led to at least temporary disability, and three percent led to death. Most of these are preventable. Another 44,000 to 98,000 deaths occur every
year due to hospital errors, many of which were preventable.
Healthcare facilities are facing increased scrutiny and regulations will hold them liable if these events are not adequately managed to reduce levels of occurrence .
