OutcomeAdvantage
Care
Management Platform
OutcomeAdvantage has developed a Care Management
Platform for hospitals and health care providers
to interact with patients and caregivers, post
discharge or outside the acute care setting. The
platform is hosted, and uses the latest
technological advances in voice portals and speech
recognition. The platform incorporates real-time
dashboards, dynamic scripting, a robust alert and
notification system as well as flexible XML and
HL7 Integration.
OutcomeAdvantage is helping set this new
standard of practice- proactive patient
interactions and communications.
Key value for the stakeholders are as follows:
Health Care Practitioners (HCPs):
The Care Coordination Platform allows doctors,
nurses, and hospitalists to better coordinate
patient care and transitions of care. Coordination
of care is crucial as patients may leave their
primary care physician (PCP) and enters a hospital
under the care of a hospitalist, and then could be
moved to a specialist or back to their home and
PCP. This Platform can be used to coordinate
patient follow-up and schedule visits, assist in
understanding discharge instructions and
medication, and help manage compliance with care
regimens. Using an outbound routine for proactive
follow-up, HCP’s are able to focus on patients
that need special attention, and leave the routine
follow up to the automated attendant.
Patients:
Patients will see improved outcomes and early
intervention from their providers/practitioners as
the Platform will help to identify those patients
confused about their medication or discharge
instructions.
Our Platform acts as an outbound triage, creating
actionable information by tracking patient data
and trends, resulting in early intervention and
better care strategies. Additionally, the platform
can identify and target key educational needs of
those patients with co-morbidities or help
patients through different phases of chronic
disease lifecycles.
Hospitals:
The Platform can drive the automation of manual
and redundant processes for hospitals employing
broad patient outreach programs, patient
screening, and patient satisfaction surveys. The
Platform provides the broadest patient outreach at
the lowest cost. It automates phone calls that
would otherwise be made by nurse or case managers
so they can devote more time to the patients that
need special attention. Hospitals can improve
patient satisfaction scores, improve quality,
reduce risks, and communicate more effectively
with patients, especially in that critical period
immediately following discharge.
Employers, Plan Providers & Payers:
Substantial savings can be derived from automating
basic processes and reducing costs while improving
outcomes. The Platform improves information flow
and communications amongst the key stakeholders in
delivery of patient care transition, reduced
hospital readmissions, improved patient compliance
and improved patient engagement. As employers,
payers and plan providers are squeezed to reduce
costs while improving care, our Platform can help
reduce costs by providing critical information,
maximizing care and outcomes. |
