The Care Coordination Platform allows doctors, nurses, and hospitalists to better coordinate patient care and transitions of care.
Coordination of care is crucial as patients may leave their primary care physician (PCP) and enters a hospital under the care of a hospitalist,
and then could be moved to a specialist or back to their home and PCP. This Platform can be used to coordinate patient follow-up and schedule visits,
assist in understanding discharge instructions and medication, and help manage compliance with care regimens. Using an outbound routine for proactive follow-up,
HCP’s are able to focus on patients that need special attention, and leave the routine follow up to the automated attendant.
Patients:
Patients will see improved outcomes and early intervention from their providers/practitioners as the Platform will help to identify
those patients confused about their medication or discharge instructions.
Our Platform acts as an outbound triage, creating actionable information by tracking patient data and trends, resulting in early
intervention and better care strategies. Additionally, the platform can identify and target key educational needs of those patients with
co-morbidities or help patients through different phases of chronic disease lifecycles.
