Mission
OutcomeAdvantage was founded to improve patient
outcomes and reduce costs by dramatically changing
the way we communicate amongst the key
stakeholders in our health care system – the
patients, hospitals, physicians, plan providers
and pharmacies. We plan to significantly improve
the way health care providers monitor, and
interact with their patients through an enhanced
communication and notification platform for
hospital/ physician/ patient relationship
management.
A major deficiency in the U.S. health care system
is the failure to coordinate patient care
activities between two or more participants
involved in patient care to facilitate the
appropriate delivery of health care services.
There are deficiencies of coordination between:
- Physicians and their patients and families
- Hospitals and their patients and families
- Primary care physicians and specialists
- Primary care physicians and emergency
departments
- Physicians and sources of diagnostic data
- Hospital-based physicians and primary care
physicians
