Hospitals and Physicians are increasing looking for critical information at their fingertips. The OutcomeAdvantage Care Coordination Platform offers a real
time view into all patient activities and critical data post discharge or from a patient satisfaction survey. This real time interactive dashboard can be customized and Key Performance
Indicators (KPI’s) can be developed for each user or role based on what the key metrics are to be monitored.
Information can be pulled from any data repository at the hospital
including the EMR, Clinical Systems, Radiology, or provide access to Continuity of Care Records to review patient discharge instructions. An HL7 or other standard industry data
integration is built to provide data from the iPIP platform back to the hospital. Physicians, Nurses, Hospitalists or Hospital Administrators can access key data relevant to
their needs through a web based portal. The dashboard portal is secure and HIPAA compliant.
Data can be seen in aggregate or drill down to patient level data. Time and date filters on the right offer discrete views into data. Filters are available to sort the data based on Hospital Facility,
Discharge Nurse, Admitting DRG, Readmitting DRG, Age, Gender, Primary Language, Unit/Floor, etc. Multiple filters may be selected for any KPI. If you want to understand the reason for readmissions
by DRG, age or Understanding of Discharge Instructions this can be done with a click of the button.
Reports are available in PDF format or exported to excel spreadsheets. Data viewing can be secured by Roles and Permissions so only certain information is made available to certain population subsets.
Root cause analysis and trending can be done quickly with alerts and notifications based on reaching certain thresholds. A Chart Builder application is available separately so hospital
IT or other users can create their own graphical views.
